Job Description

• Provide individual psychotherapy and ongoing treatment services.
• Develop, implement, and update individualized treatment plans in collaboration with a multidisciplinary care team.
• Maintain accurate, timely, and compliant clinical documentation.
• Coordinate care with facility staff, medical providers, and other healthcare professionals.
• Adhere to all federal, state, and organizational standards, policies, and regulations.
Requirements
• Active state license as one of the following: Licensed Clinical Mental Health Counselor (LCMHC) Licensed Marriage and Family Therapist (LMFT) Licensed Psychologist - PsyD/Ph.D.
• Master’s degree (LCMHC/LMFT) or Doctoral degree (Psychologist) in a relevant mental health field.
• Ability to work independently while collaborating effectively within a clinical team.
• Strong communication and clinical documentation skills.
Nice-to-haves
• Experience working with geriatric populations.
• Experience in Skilled Nursing Facilities, Assisted Living, or long-term care settings.
