Job Description

le that serves as the coordination point between CHOP-Soar and CHOP. Important features of the job are outlined below:

Responsibilities:
• Intake coordination: Manage the referral, waitlist, and intake of new potential clients across Pennsylvania's new clinics;
• Call families to schedule diagnostics, MDAs, Vinelands, tours, admissions, etc.
• Provide families with clear information and instructions, and manage them through different intake
• Work with our PA leadership to fully own the incoming pipeline of clients, including managing the intake volume and waitlist
• Meet regularly with Admissions team and leadership to review & make decisions on borderline cases and referrals
• Information management: Work between CHOP's health records system (Epic) and our internal tools to ensure client data is consistent, accurate, and up-to-date
• Collect relevant information from families and ensure it is being kept up to date in all systems
• Ensure data on client progress is consistently gathered from our clinicians and matches across systems
• Perform analyses on clinical and operational data and make recommendations to improve our care or processes
• Central communications owner: Serve as the primary point of contact between CHOP practitioners and our clinicians on a day-to-day basis
• Use Epic to send and receive clinical data about our clients to CHOP practitioners
• Serve as the go-between with our clinicians and CHOP practitioners on critical updates & requests
• Compile regular summary reports for clients that will be used in cross-team care reports
• Operational process support: Manage and improve processes in reporting, tracking, and communicating that will be critical to our success and growth in Pennsylvania

• Excellent communicator - able to have challenging conversations with families or practitioners while maintaining a positive attitude and warm, empathetic demeanor

Eligibility Requirements:
• Bachelor's degree
• Seeking long-term professional growth within Soar
• RBT level 2 or above, in good standing
• Employed by Soar for at least 6 months
• Able to work East Coast hours
