Job Description:
The Speech-Language Pathologist is responsible for comprehensive assessments in the patient's home, including the completion of skilled documentation through Homecare Homebase.
Evaluate patients and perform appropriate therapy interventions
Develop a comprehensive plan of care and coordinate multi-disciplinary care
Perform clinical documentation and visit notes
Continued communication with physician, care team members, patient and family
Comfortable working in all aspects of the home health setting
Implement and maintain SLP protocols
Manage patient treatment and outcomes directly

• Current Texas Speech-Language Pathology License
• One year of SLP experience

• Previous Home Health experience is helpful, but not required
• Current Texas driver's license
• CPR certification
