Job Description
UNCTIONS
• The Advanced Practice Provider (APP) delivers patient care in accordance with their licensure and certification.
• Diagnoses and manages both acute and chronic conditions, including diabetes, hypertension, kidney disease, and other chronic illnesses, as well as minor injuries and acute medical issues such as infections or exacerbations of chronic diseases.
• Orders and interprets diagnostic tests, including lab work, x-rays, and EKGs.
• Prescribes medications and other treatments as necessary to address identified conditions.
• Educates patients about their medical conditions, treatment plans, and expected outcomes to ensure they are well-informed and engaged in their care.
• Collaborates as part of a comprehensive, multidisciplinary team to provide high-quality, evidence-based care.
• Perform accurate and timely documentation of histories, subjective findings, physical exam, plans, prescriptions and follow up in EMR within 72 hours of the visit.
• Performs additional duties as assigned to support patient care and organizational goals.
MINIMUM REQUIREMENTS
• Graduate of an accredited PA/NP Program
• Current, valid and unrestricted State Licensure
• Valid DEA
• Current CPR - Basic Life Support from the American Heart Association
PREFERRED QUALIFICATIONS
• Three (3) years of experience working in a primary care clinic.
• ACLS / PALS are a plus.
• Bilingual in Spanish.
TOTAL COMPENSATION PACKAGE
• Great earning potential which includes a base salary plus a lucrative bonus plan.
