Job Description

Minimum Qualifications: Applicants please clearly state how you meet these qualifications on the application/resume.
• Medical Doctor Degree; Wisconsin’s Physician License; three (3) years experience as a PCP in a clinic or outpatient setting.
• State of Wisconsin and Federal DEA Certification.
• Medical Specialty License or Certification.
• Board certified or Board Eligible in Specified Area of Medical Specialty.
• Must be credentialed.

Items to be Submitted:
• Must provide a copy of diploma, license, degree, or certification upon employment.
• Employees are required to have proof of seasonal influenza vaccination; medical or religious exemption may be requested upon hire.
• Background information disclosure (BID) form
• Three (3) Current letters of references (personal, professional, and past employment).

  • Salary: $210,874/Annually - Salary Negotiable (Employees will receive 5% below the negotiated pay rate during their probationary status.)
