Minimum Education Required:
High School Diploma/GED

Responsibilities:
ul style="margin-top: 0in;"lispan style="font-size: 10.0pt;"Provide case management and skills training support to the individuals in areas such as self-care, self-preservation, receptive and expressive communication, decision-making, self-direction, activities of daily living, hygiene and grooming./span/lilispan style="font-size: 10.0pt;"Develop, implement and monitor collaborative therapeutic service plans, completing all necessary documentation and reporting significant concerns to the supervisor as needed./span/lilispan style="font-size: 10.0pt;"Assess individuals’ behaviors and complete daily data, monthly progress notes and quarterly reviews./span/lilispan style="font-size: 10.0pt;"Foster community integration and provide support to individuals to actualize this goal. /span/li/ul

Qualifications:
ul style="margin-top: 0in;"lispan style="font-size: 10.0pt;"Bachelor’s Degree; or High School Diploma and two years’ experience working with individuals with developmental disabilities./span/lilispan style="font-size: 10.0pt;"Strong interpersonal skills and ability to use good judgement./span/lilispan style="font-size: 10.0pt;"Excellent written and verbal communication skills./span/lilispan style="font-size: 10.0pt;"Basic computer knowledge./span/lilispan style="font-size: 10.0pt;"Must hold a valid driver's license, have access to an operational and insured vehicle and be willing to use it to transport clients./span/lilispan style="font-size: 10.0pt;"Strongly prefer a candidate that will have a demonstrated understanding of and competence in serving culturally diverse populations./span/li/ulp style="margin: 0px;"emspan style="font-size: 12.0pt; font-family: 'Cambria',serif; color: black;"Advocates is committed to cultivating a diverse and welcoming community where everyone feels respected and valued.
