Job Description
orting youth and families facing serious mental health challenges during critical times when they need support the most. We are in search of passionate and innovative individuals to offer care coordination, crisis intervention, counseling, and community support to families. Our values center around family-driven and youth-guided care. Services are provided in community locations, client homes, and office settings.
This position is based at our East Public Health Center located in Lombard, IL, and involves working within the community. The standard schedule involves Monday, Wednesday, and Friday from 8 AM to 4:30 PM, and Tuesday and Thursday from 10:30 AM to 7 PM, amounting to a total of 37.5 hours per week, with some potential for flexibility within the weekly schedule. Depending on your experience, the salary range for this position is as follows:
• Master's degree & license (LSW, LPC): $51,075 to $65,000
• Clinical License (LCSW, LCPC, LMFT): $59,240 to $69,940

Responsibilities:
• Provide evidence-based psychotherapeutic services to individuals, groups, or families, focusing on integrating behavioral health and primary health wellness; work in both office and community-based settings.
• Meet established productivity standards while completing documentation and exercising independent judgment in position-related decisions.
• Determine frequency, duration, and intensity of treatment while developing treatment plans.
• Provide and follow up on referrals and community resource linkages.
• Engage in team meetings to assess client progress, collaborating with coworkers, healthcare providers, and community agencies to ensure a comprehensive care plan.
• Maintain required training, licensure, and certification, upholding confidentiality and adhering to client privacy laws.
• Consult with staff on client and program matters and provide community support and case management services.
• Participate in assigned emergency response activities.
Requirements:
Master’s degree in Psychology, Social Work, or a related field, along with two years of experience working with the chronically mentally ill child and adolescent population; or an equivalent combination of training and experience is required. The ideal candidate will demonstrate an ability to collaborate with service providers and will possess an LCSW or LCPC or be eligible to take the examination.
