Position Overview
We are seeking a Full-Time Speech Therapist (SLP) to join our dynamic and supportive team. This role offers flexibility, autonomy, and the chance to provide truly personalized care. You will work directly with patients, addressing speech, language, cognitive-communication, and swallowing needs, empowering them to regain independence and achieve their goals—all in a home-based setting.

What You’ll Do

Conduct comprehensive speech, language, cognitive-communication, and swallowing assessments

Design and implement individualized treatment plans to improve communication, swallowing, and cognitive skills

Provide hands‑on therapy and practical interventions to promote progress and independence

Educate patients and caregivers with clear guidance and techniques for at‑home exercises

Track and adjust therapy plans based on patient progress, ensuring optimal outcomes

Collaborate with an interdisciplinary care team (nurses, physicians, PTs, OTs, and more) to deliver coordinated care

Maintain accurate, timely documentation to meet clinical and compliance standards

What We’re Looking For

Master’s degree in Speech‑Language Pathology from an accredited program

Active Virginia SLP license and CPR certification

2+ years of experience as a Speech Therapist (home health experience preferred but not required)

Strong clinical judgment and evidence‑based practice skills

Excellent communication and collaboration abilities

Compassionate, patient‑first approach

Experience with Home Care Home Base (HCHB) is a plus
