Job Description

d Family Services program specializes in providing support to youth and families facing serious mental health challenges during critical moments when assistance is paramount. We are searching for creative and dedicated professionals to deliver care coordination, crisis support, counseling, and community resources to families in need. Our program emphasizes family-driven and youth-guided care, ensuring we meet families where they are—be it in the community, at home, or in an office setting.

This role is based out of our East Public Health Center, located in Lombard, IL, and entails community engagement. The standard work schedule is Mondays, Wednesdays, and Fridays from 8:00 AM to 4:30 PM, and Tuesdays and Thursdays from 10:30 AM to 7:00 PM. This is a full-time position with 37.5 hours per week, offering some flexibility within the schedule. Salary will be determined based on experience, as outlined below:
• Master's degree & license (LSW, LPC): $51,075 to $65,000
• Clinical License (LCSW, LCPC, LMFT): $59,240 to $69,940

Responsibilities
• Provide evidence-based psychotherapeutic services to individuals, groups, or families, with a focus on the integration of behavioral health and primary health wellness, in both office and community settings.
• Meet established productivity standards while maintaining timely and accurate documentation and exercising independent judgment in decision-making.
• Assess and determine the frequency, duration, and intensity of treatment, developing personalized treatment plans.
• Facilitate and follow-up on referrals and community resource connections.
• Engage in team meetings to evaluate client progress and collaborate with coworkers, healthcare providers, and community agencies to ensure a comprehensive care plan.
• Maintain required training, licensure, and certification while upholding client confidentiality and compliance with privacy laws.
• Consult with staff regarding client and program-related issues, providing community support and case management as necessary.
• Participate in emergency response activities as assigned.

Requirements

Must possess a Master's degree in Psychology, Social Work, or a related field, with at least two years of experience serving the chronically mentally ill child and adolescent population, or an equivalent combination of training and experience. The ideal candidate will demonstrate the ability to work collaboratively with service providers and will be preferred to hold an LCSW or LCPC license or be eligible to sit for examinations.
