Essential Duties & Responsibilities:
• Supervise and support a team of nurses and therapists
• Review, approve, and oversee individualized patient care plans
• Ensure compliance with federal, state, and accreditation standards (e.g., Medicare/Medicaid)
• Monitor clinical documentation for accuracy, timeliness, and completeness
• Conduct case conferences and coordinate care with physicians and interdisciplinary teams
• Track patient outcomes and implement quality improvement initiatives
• Assist with staff hiring, onboarding, training, and performance evaluations
• Manage scheduling and staffing to ensure adequate patient coverage
• Address patient concerns, incidents, and clinical issues promptly
• Attend field visits to patient homes as needed

Qualifications:
• Active Massachusetts Registered Nurse (RN), Physical Therapist (PT), or Occupational Therapist (OT) license is required
• Bachelor of Science degree preferred
• 1-2 years of supervisory experience preferred
• 3 years of home health or related clinical experience
• Strong knowledge of home health regulations and reimbursement guidelines
• Proficiency in EMR/EHR systems
• Proficiency in Microsoft Word, Outlook, and Excel is required

Pay: $110,000.00 - $120,000.00 per year

Responsibilities

  • The Home Health Clinical Manager is responsible for overseeing a team of clinicians and ensuring the delivery of high-quality patient care in a home health setting
  • This role manages clinical operations, supports staff performance, and ensures compliance with all regulatory standards while coordinating patient services to achieve optimal health outcomes
  • This position follows a Monday–Friday schedule, with four days based in our Lynn office and one day working remotely
