Promote an integrated service approach, connecting members with behavioral health, long-term support services, and community resources to enhance care continuity. Facilitate interdisciplinary care team meetings and foster informal collaboration among healthcare professionals. Utilize motivational interviewing techniques and Molina clinical guideposts to educate and empower members during interactions. Identify and address barriers to care, providing coordination and support to resolve members' concerns. Offer consultation and resources to peers as needed to ensure a comprehensive support network. Be prepared for 25-40% local travel, as per state and contractual requirements. Required Qualifications: Minimum of 2 years of healthcare experience, particularly within behavioral health, or a relevant combination of education and experience. Active and unrestricted license as a behavioral health clinician, including roles such as LCSW, LPC, LPCC, LMFT, or psychologist (PhD or PsyD) as mandated by state regulations. A valid driver's license, reliable transportation, and necessary auto insurance for job-related travel. Experience working with individuals facing severe mental health challenges, emotional disturbances, substance use issues, and those in foster care. Knowledge of whole person care principles, chronic health conditions, and discharge planning coordination. Proficient in data entry and experienced in using clinical platforms. Excellent verbal and written communication skills. Proficient in Microsoft Office and related software. Preferred Qualifications: Certification as a Certified Case Manager (CCM).
