About the Role

We are excited to invite a compassionate Speech Language Pathologist (SLP) to join our Home Health team! In this role, you will have the opportunity to provide unparalleled care to pediatric patients, collaborate within a multidisciplinary team, and work closely with families to develop effective care plans.

Key Responsibilities:
• Perform comprehensive assessments and evaluations of pediatric patients to identify their physical condition, limitations, medical histories, and personal goals.
• Create, update, and implement tailored treatment plans while measuring and tracking patients' progress.
• Collaborate with physicians, patients, families, nurses, therapists, social workers, and other care team members to ensure seamless care delivery.
• Demonstrate effective communication strategies and exercises to help pediatric patients achieve their daily tasks and alleviate discomfort.
• Assess the home environment of pediatric patients and recommend modifications to better suit their needs.
• Educate families on best practices for supporting and accommodating their children’s therapeutic needs.
• Monitor and evaluate patient progress against established goals and prepare reports for healthcare providers and insurance agencies.
• Complete clinical documentation in a timely fashion using the company’s EMR system.
• Participate in meetings and case conferences to maintain coordinated and comprehensive care plans for all patients.

Qualifications:
• Master's degree from an accredited Speech Language Pathology program.
• Active and unrestricted Speech Language Pathologist license; or the ability to obtain one.
• Preferred: At least one year of pediatric speech therapy experience.
• Compliance with continuing education requirements.
• Valid driver's license and auto insurance required.
• Current CPR certification.

Compensation:

{@}$100 per evaluation / $65.00-$75.00 per follow up

Additional Information:

This is a 1099 position offering options to work PRN, part-time, or full-time. You can choose your hours, work locations, and preferred treatment areas.
