Seeking a Family Medicine Physician in San Jacinto, California to provide full scope outpatient primary care in a collaborative community health environment. This position offers the opportunity to deliver meaningful care focused on prevention, chronic disease management, and improving access to care for diverse patient populations.
Job Overview
The Family Medicine Physician provides comprehensive outpatient primary care for patients of all ages, including examinations, diagnosis, treatment planning, preventive screenings, and minor procedures. Physicians work closely with advanced practice providers, nurses, and care teams to deliver coordinated, patient centered care.
Responsibilities include managing acute and chronic conditions, supervising clinical staff, coordinating referrals and specialty care, maintaining accurate documentation, and supporting quality improvement initiatives. Providers also participate in after hours call rotation and Patient Centered Medical Home initiatives focused on improving outcomes and care access.
Compensation
• $300,000 to $310,000 annual salary
• $30,000 sign on bonus
• Paid half at hire and half at six months

Qualifications
- •MD or DO from an accredited medical school
- •Active California medical license
- •Board Certified in a primary care specialty
- •Current DEA registration and BLS certification
- •Experience in primary care, preventive care, minor procedures, gynecologic care, and urgent care management
- •Ability to supervise Nurse Practitioners, nurses, and clinical staff
- •Strong communication and patient care skills
- •Availability for occasional evening or weekend coverage
