Job Profile Summary:

The Population Health Clinical Associate – Ambulatory Care plays a key role in advancing high-quality, equitable patient care across multiple outpatient practices. This position blends hands-on clinical support with care coordination, documentation management, staff support, and workflow optimization to improve patient outcomes and operational efficiency. This role partners closely with providers, clinical teams, and population health leadership to support continuity of care, close care gaps, and enhance the overall patient experience. The Clinical Associate serves as a flexible, team-based resource across sites, helping standardize workflows and support evolving population health initiatives in a patient-centered, non-judgmental environment.

Required Qualifications And Skills:
• Minimum of 3 years of experience in an ambulatory or outpatient healthcare setting involving clinical support, care coordination, or clinical operations
• Strong verbal and written communication skills
• Demonstrated ability to work independently across multiple practice locations
• Strong organizational, prioritization, and problem-solving skills
• Comfort navigating electronic medical records and managing clinical documentation

Preferred Qualifications And Skills:
• Medical Assistant experience or equivalent hands-on clinical experience
• Familiarity with population health initiatives, care gap closure, and quality workflows
• Experience with referrals, prior authorizations, and care coordination processes
• Experience supporting staff onboarding, training, or workflow improvement
• Knowledge of medical terminology and ambulatory billing workflows

Essential Job Functions:

Clinical Support and Patient Care
• Assist with panel management activities, including reviewing patient lists, identifying patients overdue for care, and supporting outreach efforts
• Reinforce care plan adherence through patient education, follow-up communication, and coordination with the care team
• Support targeted initiatives (e.g., chronic disease management, screenings, and transitions of care) to improve overall patient outcomes
• Provide clinical support as needed (e.g., vitals, rooming, basic procedures) to maintain workflow efficiency across practices

Care Coordination & Documentation
• Support referrals, prior authorizations, and follow-up coordination in collaboration with clinical teams
• Review, interpret, and route clinical documentation accurately within the EMR
• Assist with patient outreach, follow-up communication, and coordination between providers, staff, and patients
• Ensure accurate and timely documentation to support quality measures and population health reporting

Ambulatory Operations & Workflow
• Support efficient patient flow and standardized clinical workflows across assigned practices
• Assist with provider schedule management and template optimization
• Identify workflow gaps and partner with leadership to implement process improvements
• Coordinate inventory, ordering, and maintenance of clinical supplies

Team Support, Training & Coverage
• Serve as a clinical resource for medical assistants and support staff across practices
• Assist with onboarding, training, and ongoing support for new and existing team members
• Provide cross-site coverage and support staffing needs to maintain operational continuity

Quality, Safety & Patient Experience
• Promote a safe, respectful, and inclusive environment for patients and staff
• Support quality improvement initiatives focused on care gap closure, patient engagement, and health outcomes
• Partner with population health and practice teams to improve performance on key quality measures
• Contribute to a positive patient experience through compassionate, patient-centered interactions

• Clinical responsibilities include rooming patients, taking vitals, procedures, prescription renewals, triage of patient phone calls, review charts, lab test results, scheduling patients, responding to provider messages
• Commitment to a safe patient experience and quality improvement
• Act as the medical assistant mentor and leader for any clinical/administrative questions
• Provide back up support for the medical assistant team, which includes all functions of a medical assistant
• Assist in the supervision of work and performance of medical assistants, including input for performance reviews
• Oversee and train new and existing staff along with Practice Manager and Office Coordinator
• Support staff with Prior authorizations and referrals
• Review, interpret, and manage clinical documentation
• Perform other administrative tasks and clinical care as assigned
• Coordinate the inventory, ordering and supply stock maintenance of medical supplies
• Work collaboratively with office staff on programs to improve patient outcomes

Salary Range:$19.85-$30.05
