Job Description
nate Family Medicine Physician to join our Primary Care team. The ideal candidate will provide comprehensive outpatient care to patients of all ages, from pediatrics to geriatrics, and will promote wellness and preventative care across the continuum of life.
Key Responsibilities
• Diagnose and treat acute and chronic illnesses for patients of all ages.
• Provide preventive care and health education.
• Coordinate patient care with specialists, hospitals, and ancillary services.
• Maintain accurate and up-to-date medical records via EMR.
• Participate in quality improvement initiatives and team-based care models.
• Foster positive relationships with patients, families, and clinical support staff.
• Adhere to clinical and ethical standards in accordance with Vista's mission and values.
Qualifications
• MD or DO degree from an accredited medical school.
• Board Certified or Board Eligible in Family Medicine.
• Active Illinois medical license or eligibility to obtain one.
• Current DEA and CPR certifications.
• Excellent communication and interpersonal skills.
• Commitment to serving a diverse, multicultural patient population.
