Job Description
orting youth and families facing serious mental health challenges during critical moments when they need it most. We are looking for compassionate and innovative individuals to deliver care coordination, crisis intervention, counseling, and community support. Our approach is rooted in family-driven and youth-guided care, ensuring that we meet the unique needs of those we serve.
This role is based at our East Public Health Center in Lombard, IL, involving outreach and collaboration within the community. The typical work schedule is Monday, Wednesday, and Friday from 8:00 AM to 4:30 PM, and Tuesday and Thursday from 10:30 AM to 7:00 PM. This is a full-time position, totaling 37.5 hours per week, with some flexibility available within the schedule. The salary for this role varies based on experience:
• Master's degree & license (LSW, LPC): $51,075 to $65,000
• Clinical License (LCSW, LCPC, LMFT): $59,240 to $69,940

Key Responsibilities:
• Deliver evidence-based psychotherapeutic services to individuals, groups, or families, focusing on the integration of behavioral health and primary health wellness, in both office and community settings.
• Meet established productivity standards while maintaining accurate documentation.
• Assess treatment frequency, duration, and intensity, and develop personalized treatment plans.
• Provide and follow up on community resource linkages and referrals.
• Engage in team meetings to assess client progress and collaborate with coworkers and community agencies for comprehensive care.
• Maintain required training, licensure, and confidentiality while adhering to client privacy laws.
• Consult with staff regarding client and program issues, providing community support and case management services.
• Participate in emergency response activities as assigned.
Qualifications:
Candidates must possess a Master's degree in Psychology, Social Work, or a related field, along with two years of experience working with chronically mentally ill children and adolescents, or an equivalent combination of training and experience. Preferred candidates will demonstrate the ability to work collaboratively with service providers and hold an LCSW or LCPC license or be eligible to test.
