 Advanced Practice Providers and Care Teams to deliver comprehensive primary care in a modern, well-supported clinic.

Practice Info
• Outpatient role within a team-based primary care model
• Modern, well-supported clinic focused on preventive care, chronic disease management, and long-term patient relationships
• Team-based care model that allows you to focus on complex decision-making
• Expanded patient access model supported by APPs, including extended hours, virtual visits, and same-day care

Responsibilities
• Deliver comprehensive primary care
• Supervise Advanced Practice Providers

Shift & Schedule

Requirements
• Board-certified or board-eligible physicians, with intent to obtain certification; Family Medicine preferred, Internal Medicine considered
• Passion for whole-person, family-centered primary care
• Comfortable working in a team-based model supervising Advanced Practice Providers
• Strong clinical, communication, and patient-engagement skills
• BLS Required
