• Manages patient medical care including diagnosis, treatment and follow-up
• Maintains appropriate documentation on patient clinical records
• Maintains office hours as negotiated, including evening and Saturday hours, as required
• Participates in continuing medical education to meet standards of retaining license and Board Certification
Education/Experience:
• Graduate of Accredited medical school with degree of Doctor of Medicine or Doctor of Osteopathy Completion of Accredited residency program
Certification/Licenses:
• Unrestrictive Physician License through State of Illinois Department of Financial and Professional Regulations.
• Current State of Illinois Control Substance License
• Board Certified or Board Eligibility
• Current DEA certificate
• BLS Certification
