s to support students in particular grade cohorts. A maximum of 25% of our student population are typically enrolled in our PHS program. Our Post High School students participate in dual enrollment college coursework, plus offsite & onsite internships.
We are currently recruiting for a Transition Counselor/Teaching Assistant to join our team for a Spring 2026 start date, dependent on confirmation of increased student enrollment.We offer a unique and intensive year-round academic and therapeutic programming in a state of the art building. Our students are highly supported by a full clinical staff working in multidisciplinary teams that consists of Psychologists, Occupational Therapists, Speech Therapists, Board Certified Behavior Analysts, Principals, Mentor Teachers and Special Educators, Reading and Math Specialists, and Directors. We have a low student to staff ratio.Job Description: The Transition Counselor is responsible for providing direct instruction and support to students across a variety of school-based and community-based settings for students age 18-22 within our post-high school program. As an integral member of the Transition Team, the Transition Counselor works collaboratively with a variety of staff (transition, therapeutic, behavioral) to increase student success beyond the classroom.
Counselors work with students to explore and integrate reasonable accommodations throughout all aspects of adult life -- employment, education, and community/independent living. It is the Transition Counselors responsibility to provide regular ongoing support to program administrators in planning and facilitating student programming.Responsibilities Include, but not limited to: - Deliver individualized instruction in the areas of career awareness, job readiness, socialization, safety, travel training, self-help skills, functional academics, self-advocacy, and self-determination.- Instruct students in a variety of community, employment, and education settings to support their post-secondary skill development.- Identify, develop, and maintain on-campus and community job sites.- Responsible for collaborating with program administrators on identifying appropriate skills prerequisite to a job placement and identifying work sites for students.- Collect and summarize performance data on an ongoing basis.- Collaborate and consult with program administrators to ensure students receive individualized, functional supports on an ongoing basis.- Ability to accurately analyze situations and adopt an effective course of action.- Demonstrate team leadership skills to include initiative, positivity, problem-solving, and flexibility.- Facilitate regular, ongoing communication with program administrators.Qualifications: - Bachelors degree or higher- Experience with job coaching and/or vocational experience strongly preferred- Previous experience working with students with Autism, developmental, psychiatric, and/or behavioral issues preferred- Experience with behavior management preferred- Suitable driving record requiredHours: Full time 7:30am-3pm, plus 5 hours/week on or offsite for planningMilestones Provides Excellent Benefits Including: - Competitive salary- 6 weeks of paid vacation, 8 sick days, and 2 personal days in addition to scheduled holidays.
