Our multispecialty team includes board-certified physicians in Family Medicine and Nephrology, working collaboratively with Advanced Practice Providers to serve medically complex and geriatric populations.

Position Overview
We are seeking a compassionate and clinically strong Advanced Practice Provider (NP/ARNP) to provide comprehensive medical care to residents in Skilled Nursing and Long-Term Care facilities across WA, OR, MT, and AK.
This role is ideal for a provider who:
• Is comfortable managing medically complex patients
• Enjoys practicing with autonomy while collaborating with physicians
• Is passionate about geriatric and post-acute medicine
• Thrives in a multidisciplinary environment
Key Responsibilities
Clinical Care
• Perform comprehensive history and physical examinations
• Evaluate, diagnose, and manage acute and chronic conditions
• Develop individualized treatment plans
• Order and interpret diagnostic tests and labs
• Prescribe medications and therapies within scope of practice
• Provide post-hospital follow-up care
• Address cognitive, behavioral, and functional changes
• Educate patients and families on treatment plans
Skilled Nursing & Regulatory
• Complete admission assessments and required documentation
• Conduct routine and medically necessary follow-up visits
• Participate in interdisciplinary care plan meetings
• Support initiatives to reduce hospital readmissions
• Ensure compliance with CMS and state regulations
• Maintain accurate and timely EMR documentation (labs signed same day received)
Collaboration
• Work closely with assigned collaborating physician
• Partner with nursing, therapy, and facility teams
• Communicate effectively with families and caregivers
• Participate in multidisciplinary patient management
Qualifications
Required
• Graduate of an accredited NP/ARNP program
• Active, unrestricted license in WA, OR, MT, or AK
• National board certification (or board eligible by start date)
• DEA registration and prescriptive authority
• NPI number
• Eligible for Medicare/Medicaid participation
• Current BLS certification
• U.S. citizen or appropriate work authorization
Preferred
• Experience in Skilled Nursing, Long-Term Care, Geriatrics, Internal Medicine, or Family Medicine
• Knowledge of CMS guidelines and regulatory standards
• Experience managing complex chronic conditions
• New graduates encouraged to apply
