Job Description
Support Professionals/Caregivers who are driven by a desire to make a substantial difference in a part-time role.
Position Details:
Schedule: Monday-Sunday, any where from 6am-10pm
Location: Various community settings in and around Tracy.

Your Role:
Implement and manage daily schedules to ensure participants engage in enriching activities both at home and in the community.
Assist with mobility, personal care, hygiene, and daily living tasks to promote independence.
Transport participants using company vehicle to various community locations, ensuring safe and timely arrival.
Support participants in physical therapy exercises and help prepare meals, promoting participant involvement.
Manage shopping needs either independently or as a supportive companion and perform light housekeeping tasks.
Serve as a friendly and supportive companion, ensuring a positive and nurturing environment.
Respond promptly and efficiently to emergencies and unusual incidents.
Document activities daily and report back to the Manager/Administrator.
Perform additional tasks as assigned by management, contributing to the smooth running of our program.
Who You Are:
Experienced in supporting individuals with disabilities and understanding of their unique needs.
In possession of a valid driver's license, with a clean DMV record and comfortable using company provided vehicle for participant transport.
Able to pass a federal background check (costs reimbursed).
Certified in CPR with knowledge of first aid and emergency response.
A compassionate communicator with outstanding interpersonal skills, capable of building trust and rapport.
Ethically strong, with excellent time management skills.
Physically fit to meet the demands of the role, must be able to walk up to 2 miles.
Educational Qualifications:
High school diploma or equivalent; further education or training in care is advantageous.
